Referral letters and information
Please help us to help you. We appreciate the referrer providing background information with the referral. This may include (where applicable)—
· Job or duty description · Injury or incident report(s) or investigation(s) · Worker’s compensation application(s) · Sick leave history · Relevant medical information or reports · Specific questions you may have – for example, you may wish us to comment on—
1. Diagnosis or diagnoses 2. The presence of any pre-existing pathology & aggravation of this 3. Causative factors & relationship to work (or an alleged incident) 4. Work capacity & function 5. Psychological factors which may be relevant or may impact on treatment or recovery 6. Stability of the condition & prognosis 7. Potential for future aggravation 8. Ongoing effects of the work related injury and any permanent sequelae (or permanent impairment) 9. The presence of any inconsistency 10. Recommendations regarding: medical treatment & investigations, further assessments, physical rehabilitation, vocational rehabilitation and other recommendations
Fees
At QVHS we believe fees for health services should be fair and reasonable; for this reason we follow the fees published by the WorkCover Authority of NSW, VWA, TAC, Q-Comp and Comcare Australia for all applicable services. Services not covered are billed according to published AMA fees.
Consumable items (such as vaccines, dressings and medicines) are billed at cost price plus 5-10%.
Please note some services attract GST; these include pre-employment assessments, medico-legal assessments and medical reports. Consultations for treatment or rehabilitation based services are generally GST exempt. Please ask us if you need further clarification about GST and our services.
Quality assurance
Our quality procedures ensure that reports are dispatched promptly; all reports (including medico-legal reports) are completed within one to three days. Brief rehabilitation reports and pre-employment assessment reports are generally available on the day of the assessment.
Reports may be delayed pending receipt of further information or results of medical tests. Where this occurs, an interim report may be issued. |
